‘Out of Office’ Internal Medicine Services
Thank you for choosing Kimberlee Terry, M.D. as your health care provider. Dr. Terry and staff are committed to providing the most convenient and successful treatment options for their patients. We realize that an office visit is not always possible or necessary. Below is a list of additional services and fees should you have any out of office internal medicine needs. Please note we do not accept insurance for these services and payment must be made upfront. We will provide documentation for you to submit to your insurance for reimbursement upon request. Charges reflect standard and usual fees for the area.
Email and Telephone Consultations
E-mail and phone consults are available by request only and based upon availability. Each phone and e-mail consult will be charged $25-$75 based on complexity. Email correspondence must be through www.reachmydoctor.com, a secure email server that is HIPPA compliant.
Medication Refills, Referrals and Testing
Medication refills, orders for testing and referrals not requested at the time of your visit will be assessed $20 per call or email. Please keep in mind some medications may be refilled for 12 months after an annual exam, but many medical conditions require more frequent monitoring. Requests for testing or referrals may require an office visit or remote consultation.
Copies and Release of Medical Records
All requests for copies or faxing of medical records will be charged $15-$30 per request. Charge is based upon size, type of copy request, number of copies requested and urgency. No charges will be made for faxing records for referrals but copies for referrals will be charged a fee as above.
A physician is always available for urgent after-hour needs at (303)727-0457. This service is not intended for non-urgent matters such as medication refills or test results and should wait until regular office hours.
